Re-endodontics of tooth 2.4 using RetreatAll\u00ae files and Z-Activator\u00ae in irrigation<\/span><\/span><\/span><\/h2>\n

Dr. Marc <\/span>Llaquet<\/span> presents<\/span> in <\/span>this<\/span> clinical<\/span> case a <\/span>re-endodontic<\/span> treatment<\/span> of<\/span> tooth<\/span> 2.4 <\/span>using<\/span> RetreatAll<\/span> files <\/span>to<\/span> remove<\/span> gutta<\/span>-percha and <\/span>the<\/span> ultrasonic<\/span> activation<\/span> of<\/span> Z-<\/span>Activator<\/span> in <\/span>the<\/span> irrigation<\/span>.<\/span><\/span>\u00a0<\/span><\/span><\/span><\/h4>\n

Clinical<\/span> case: <\/span>Root<\/span> canal <\/span>treatment<\/span> of<\/span> a <\/span>tooth<\/span> with<\/span> a<\/span> previously<\/span> treated<\/span> acute apical <\/span>abscess<\/span>.<\/span><\/span>\u00a0<\/span><\/span><\/span><\/span><\/strong><\/h3>\n

Diagnosis<\/span><\/strong><\/h3>\n

[\/vc_column_text][vc_column_text]The patient comes with discomfort “when biting” in the second quadrant. In the clinical examination we observed numbness of the mucosa at the level of the left second premolar, with discomfort on palpation and percussion.\u00a0<\/span>\u00a0<\/span><\/p>\n

It\u2019s observed a large apical lesion in tooth 24 (which has already been endodontically treated), both in the periapical radiograph and in the CBCT.\u00a0<\/span>\u00a0<\/span><\/p>\n

Final diagnosis: Previously treated tooth with acute apical abscess.<\/span>[\/vc_column_text][vc_single_image image=”4275″ img_size=”full” add_caption=”yes” alignment=”center” css_animation=”none”][\/vc_column][\/vc_row][vc_row full_width=”stretch_row”][vc_column][vc_empty_space][vc_column_text]<\/p>\n

Treatment<\/span><\/strong><\/h3>\n

[\/vc_column_text][vc_column_text]Dr. Llaquet decides to perform orthograde retreatment with the Zarc RetreatAll\u00ae<\/span><\/span><\/a> file system. This simple system of 2 rotary files allows easy removal of coronal and apical gutta-percha, as well as other materials in the canals. With the canal flooded with sodium hypochlorite, the doctor starts by removing the coronal gutta-percha from the 2 canals with the ZR1 file (30\/04, 21 mm) at 500 rpm. With the ZR2 file (25\/04 25 mm) he finishes removing all the sealing material up to the length calculated in the CBCT.\u00a0\u00a0<\/span>\u00a0<\/span><\/p>\n

We use the 8K and 10K hand files to permeabilize the palatal canal and check with the E-Connect S apex locator that both canals meet at 5 mm from the constriction. We instrument both canals with the ZR2 at 25mm.<\/span>\u00a0<\/span>[\/vc_column_text][vc_single_image image=”4278″ img_size=”full” add_caption=”yes” alignment=”center”][vc_column_text]Dr. <\/span>Llaquet<\/span> irrigates<\/span> with<\/span> 17% EDTA <\/span>for<\/span> 30 <\/span>sec<\/span>\/<\/span>duct<\/span> and 5.25% <\/span>sodium<\/span> hypochlorite<\/span> for<\/span> 1 min\/<\/span>duct<\/span> and <\/span>activates<\/span> it<\/span> ultrasonically<\/span> with<\/span> Zarc’s<\/span> Z-<\/span>Activator\u00ae<\/span><\/span><\/span><\/a><\/span><\/span>\u00a0system<\/span>, <\/span>which<\/span> achieves<\/span> a <\/span>unidirectional<\/span> flow<\/span> of<\/span> the<\/span> liquid<\/span>. In <\/span>addition<\/span>, <\/span>the<\/span> movement<\/span> of<\/span> the<\/span> tip<\/span> multiplies<\/span> the<\/span> movement<\/span> of<\/span> the<\/span> liquid<\/span> inside<\/span> the<\/span> root<\/span> canal, <\/span>significantly<\/span> amplifying<\/span> the<\/span> effectiveness<\/span> of<\/span> the<\/span> cleaning<\/span>. <\/span>Finally<\/span>, <\/span>he<\/span> seal<\/span>s<\/span> the<\/span> root<\/span> canal <\/span>system<\/span> using<\/span> the<\/span> continuous<\/span> heat<\/span> technique<\/span> with<\/span> the<\/span> Fast<\/span> Pack and <\/span>Fast<\/span> Fill<\/span> system<\/span>.<\/span><\/span>\u00a0<\/span>[\/vc_column_text][vc_single_image image=”4280″ img_size=”full” add_caption=”yes” alignment=”center”][vc_single_image image=”4282″ img_size=”full” add_caption=”yes” alignment=”center”][\/vc_column][vc_column][\/vc_column][\/vc_row][vc_row full_width=”stretch_row” bg_type=”bg_color” bg_color_value=”#e8e8e8″][vc_column width=”1\/4″][vc_single_image image=”4284″ img_size=”210×210″ style=”vc_box_circle_2″][\/vc_column][vc_column width=”3\/4″][vc_column_text]<\/p>\n
